Operative words

WHEREAS, resumption of services for LTCFs requires a phased in approach, based on each [facility's outbreak status, testing and staffing]... [facilities that cannot meet the criteria revert] to the maximum restrictions of Phase zero (0), as described herein. Phases per this Directive: Phase 0: Any facility with an active outbreak of COVID-19, as defined by the [Communicable Disease Service] Manual Chapter, any facility that cannot attest to criteria to advance phases, and all [facilities at the outset]... Phase 1: Facilities that never had an outbreak or that concluded an outbreak... Phase 2... Phase 3... [with visitation, communal dining, group activities and non-essential personnel entry expanding by phase, weekly staff testing and attestation].
