PIN 21-44-ASC notifies all Adult and Senior Care (ASC) licensees of the COVID-19 [vaccination requirement in the State Public] Health Officer Order of September 28, 2021.... Pursuant to the State Public Health Officer Order of September 28, 2021, all workers [in adult and senior care facilities must have their first dose of a one-dose regimen or their] second dose of a two-dose regimen by November 30, 2021.... Workers may be exempt from the vaccination requirements only upon providing [a declination form based on religious beliefs or qualifying medical reasons]... the unvaccinated exempt worker must meet the following requirements... Testing must occur once weekly for such workers.
