Operative words
A health officer or the Department... shall, by written order, establish such isolation or quarantine measures as... necessary to prevent or control the spread of the disease.... Any person who is responsible for the care, custody, or control of a person who is ill or infected with a communicable disease shall take all measures necessary to prevent transmission of the disease to other persons.

What it required
On report of a communicable disease (COVID-19 was reportable throughout the period) the health officer or Department had to issue written isolation or quarantine orders, could quarantine exposed persons for the incubation period and restrict access to isolated persons, and any person responsible for the care, custody or control of an infected person had to take all measures necessary to prevent transmission.

N.J.A.C. 8:57-1.15 (Enforcement) was read on LII: it recites fines under N.J.S.A. 26:4-129, -130 and 26:1A-10 for reporting failures by physicians, facilities, laboratories, health officers, child care centers, schools and camps.
